Optional Website Measurement
You can allow or decline optional measurement using “Cookie preferences” at the bottom of any page. Your choice does not affect access to care or the Call and Directions links.
If you allow measurement, Google Analytics receives website page URLs, page titles and actions such as tapping Call, Directions or Save Your Spot. Some pages describe health conditions. Advertising personalization and Google Signals are disabled in this site's measurement code.
Your browser remembers your measurement choice for up to 180 days. It may store neutral campaign and advertising click identifiers for up to 90 days and pass those identifiers to Solv when you follow a booking link. Solv's own privacy practices apply to information you provide there.
Declining stops this site's optional Google Analytics events, clears this site's Google Analytics cookies and removes stored campaign identifiers. Website actions do not establish a completed booking or a patient arrival.
Separate Meta page measurement choice
Optional Meta page measurement is a separate choice in Measurement choices. A previous Google measurement choice does not authorize Meta. If you allow it, this site sends only a general PageView on the home and general emergency-care pages. The browser request contains the public dataset identifier, event name and a random event ID; it sends only our website origin as referrer. A matching server request sends the same event ID, event time and general page address (home or emergency-access page), plus your network address and browser information. These requests contain no query, title, campaign or click identifier, symptom, form value, booking action or patient information.
The browser and server requests share your network address and browser information with Meta. Matching event IDs help Meta avoid counting the same visit twice. No Meta JavaScript runs here, and this cross-origin request omits browser credentials. This site does not enable Meta measurement on condition, article, question, location, booking, billing or form routes, advanced matching, patient audiences or patient-level outcomes.
Your separate Meta choice is remembered for up to 180 days. Declining or withdrawing stops future requests and cancels requests still pending where possible. It cannot recall requests already received by Meta. PageView does not establish a completed booking or an arrival.

Bellaire ER is committed to protecting your health information. We are required by law to maintain the privacy of your protected health information (PHI), provide you with this notice of our legal duties and privacy practices, and notify you in the event of a breach of your unsecured PHI.

- Right to Access: You may request copies of your medical records. We may charge a reasonable fee for copying.
- Right to Amend: You may request corrections to your health information if you believe it is inaccurate or incomplete.
- Right to Restrict: You may request restrictions on how we use or disclose your information for treatment, payment, or operations.
- Right to Accounting: You may request a list of disclosures we have made of your health information.
- Right to Confidential Communications: You may request that we communicate with you in a specific way or at a specific location.
- Right to a Paper Copy: You may request a paper copy of this notice at any time.
How We Protect Your Information
- Electronic health records are secured with encryption and access controls
- Staff undergo HIPAA training annually
- Physical records are stored in secure, limited-access areas
- We conduct regular security assessments
Breach Notification
In the event of a breach of your unsecured protected health information, we will notify you as required by federal and state law. Notification will be provided without unreasonable delay and no later than 60 days after discovery of the breach.
Changes to This Notice
We reserve the right to change this notice and make the new provisions effective for all information we maintain. Current notices will be posted in our facility and on our website.
