Facility Notice Regarding Insurance

Important: Bellaire ER is a freestanding emergency room. We may be considered out-of-network by some insurance plans. Federal law protects you from unexpected bills for emergency services.
About Bellaire ER
Bellaire ER is a licensed freestanding emergency medical care facility operating 24 hours a day, 365 days a year. We are staffed by board-certified emergency medicine physicians and provide full-service emergency care including on-site laboratory, X-ray, CT scan, and ultrasound.
Your Rights Under the No Surprises Act
Effective January 1, 2022, the No Surprises Act provides important protections for patients receiving emergency medical care:
- Emergency Services: You cannot be charged more than your in-network cost-sharing amount for emergency services, even if the facility or provider is out-of-network.
- No Balance Billing for Emergencies: Out-of-network providers and facilities cannot bill you more than your in-network cost-sharing for emergency services.
- Good Faith Estimate: Uninsured or self-pay patients have the right to receive a good faith estimate of expected charges before receiving non-emergency services.
- Consent Required: For non-emergency services at out-of-network facilities, providers must give you a written notice and obtain your consent before treatment.
Insurance Information
Bellaire ER accepts most major insurance plans. However, some plans may consider our facility out-of-network. If your plan considers us out-of-network:
- You are protected by the No Surprises Act for emergency services
- Your cost-sharing (deductible, copay, coinsurance) will be based on in-network rates
- We will work directly with your insurance company on billing
- You will not receive a surprise balance bill for emergency care
Good Faith Estimate for Uninsured Patients
If you are uninsured or choose to self-pay, you have the right to receive a Good Faith Estimate of expected charges. You can request this estimate before scheduling a non-emergency service.
If you receive a bill that is at least $400 more than your Good Faith Estimate, you may dispute the bill through the patient-provider dispute resolution process.
Texas Facility Disclosure
As required by the Texas Health and Safety Code, we disclose that Bellaire ER is a freestanding emergency medical care facility. We are not a hospital emergency department. Our facility fees and physician fees may differ from those charged by hospital-based emergency rooms.
